Hypertension is a chronic cardiovascular condition requiring continuous monitoring, lifestyle modification, and medication adherence. Community pharmacists are wellpositioned to provide longitudinal care for clients. This case reports the management of a 59-year-old male with uncontrolled hypertension presenting to a community pharmacy for a refill of Amlodipine 10 mg daily. The pharmacist identified poor adherence, unhealthy dietary practices, and amlodipine-induced ankle edema. Over four encounters spanning three months, interventions included medication review, counseling on diet and lifestyle, adherence support, and referral to a physician. Therapy was adjusted to Amlodipine 5 mg + Hydrochlorothiazide 25 mg daily. The client achieved progressive improvement in blood pressure (from 158/94 mmHg at baseline to 128/82 mmHg), resolution of ankle edema, weight reduction, improved diet, decreased alcohol 1intake, and excellent adherence. This multi-encounter case highlights the essential role of community pharmacists in optimizing hypertension management through ongoing monitoring, lifestyle and nutrition counseling, and collaboration with physicians.
